The way that Eden Health’s product is designed ensures longitudinal care through a variety of ways. First, whenever a patient inputs a request or question, their assigned care team takes care of the issue. If they are scheduling an appointment, they have the choice to choose their usual doctor, or the first available. Even if they do choose the first available, the patient’s care team is alerted and results are shared after. This way, every person truly has a team looking after their entire health journey. With the PCMH certification, we are able to keep all of a patient’s records in one place (even those from doctor’s outside of Eden Health). We then become their digital medical home - a place they can rely on to be the starting point for all of their care. The result of this means that we are able to streamline care, make sure a patient's entire medical history is taken into account, and as a result, we see a 39% increase in preventative care utilization. Because of this, medical costs go down.
