Tech Brief
The SHOW program is designed specifically to bring healthcare to the unsheltered, increasing healthcare equity and improving patient outcomes. SHOW mobile units operate on a “no patient turned away” basis and do not charge patients for services or treatments. While these units are stationed and equipped to care for the city’s street homeless population, we will not turn away anyone we can help. Because of the (many) stigmas people experiencing homelessness face, it’s rare for unsheltered patients to approach our buses if they haven’t built a relationship with us yet. This is part of what makes our roving teams crucial to this effort. Our roving teams, which comprise one advanced practice provider (APP), one paramedic, and one social worker, are equipped with backpacks full of medical supplies, medication, hygiene kits, food and water, and naloxone kits. They “rove” on foot to find unsheltered New Yorkers in need of care. Roving teams perform wound care, irrigations, sutures, give antibiotics among other medications, and provide other essential procedures. These roving teams are focused on engaging and building relationships with those living on the street, which in time increases those individuals’ willingness to accept care. This ensures that resources directly serve the program’s target population. Through SHOW, we can directly improve health equity, access to care and resources, and provide quality-of-life services to our most vulnerable populations.

The SHOW Program is uniquely dedicated to increasing healthcare access among vulnerable populations. It is the only citywide program combining outreach and street medicine to provide unsheltered New Yorkers with testing and vaccinations, medication, primary care, mental healthcare resources, harm reduction services, and external resources they can use to find food, clothes, a place to stay, and other critical services. What sets us apart is the combination of our dedicated clinical staff and proprietary technology, along with NYC H+H’s expansive resources and reach as the largest public health care system in the U.S. At DocGo, our software platform is at the heart of everything we do. Our integration with some of the largest electronic health record (EHR) systems in the nation allows us to seamlessly communicate crucial patient details to providers and enhances all aspects of care logistics. HealthPoint is a HIPAA-compliant, patient-facing app that enables turnkey registration, reporting and data storage, and features HL7 integration with the CDC for vaccination reporting and Tableau integration to allow robust data reporting to our customers. Patients with smartphone access can use Healthpoint to access their information. Once SHOW patients are registered in HealthPoint, we can leverage our embedded telehealth platform to consult with a physician and prescribe medications at a local pharmacy. For patients that we refer to H+H Primary Care Safety Net Clinics, we use a digital tracking tool to follow up with patients and monitor their status, ensuring that the hardest-to-reach cases get the care they need.
Validation
While we haven’t conducted a study, we carefully track all patient encounters. Since the project launched in April 2021, SHOW mobile units and roving teams have provided over 150,000 patient encounters, focusing on New Yorkers experiencing homelessness. To date, those services include 67,000 COVID-19 tests, 12,800 medical consultations, 7,000 vaccinations and more than 35,000 social work engagements. By actively bringing treatment to individuals not prone to seek treatment, we can treat medical issues before they become full-blown crises, which reduces costly emergency room visits and hospital admissions.
